The drop box will close at 11:59 pm. 2 points will be deducted every day that the paper is late starting after 11:59 pm on the due date. No hard copies will be accepted. No hand written copies will be accepted. No exceptions.
Using the Heritage Assessment tool, that is available on D2L and found on page 2 below, you are to complete a heritage assessment. The Microsoft Word document on D2L allows you to fill in your answers to each question. This assessment must be completed with another individual. This individual must be of a different cultural/ethnic background from you. You are to complete all of the questions and then write a summary. In your summary, you must address the following questions:
1.        Who you selected and why you selected that individual.
2.       What stood out for you in doing this assessment?
3.       Was there anything that surprised you?
4.       Describe some ways this individual is different from you and how is he or she the same?
5.       Imagine that this individual is one of your patients.
6.        How would the information you obtained influence future health care encounters? Would you consider this individual to be more culturally traditional or acculturated and why?

Heritage Assessment Tool  (From Cultural Diversity in Health and Illness, Rachel Spector, 2004)
This set of questions is to be used to describe a given client's ethnic, cultural, and religious background. In performing a heritage assessment it is helpful to determine how deeply a given person identifies with his or her traditional heritage. This tool is most useful in setting the stage for assessing and understanding a person's traditional health and illness beliefs and practices and in helping to determine the community resources that will be appropriate to target for support when necessary. The greater the number of positive responses, the greater the degree to which the person may identity with his or her traditional heritage. The one exception to positive answers is the question about whether or not a person's name was changed.
