formula to determine body surface areas

Please help me with my math for health care class

please show the work so I can understand how to answer the question thank you

Formula to determine body surface areas:

 Can use: BSA = (W0.425  X H0.725) X 0.007184

 Can use: BSA = sqrt(W x H/3600)

Note: Weight must be in kilograms and Height must be in centimeters

1) You are to infuse heparin 25000 U in 250 mL NS at 10.6 mL/hr. What is the concentration of heparin solution? When you clear the pump you note that 67 mL have been infused. How much heparin has been given? Show your work.

 2) A client weighs 154 lbs and is 5 feet 4 inches. What is the client’s BSA? Use the formula to determine the BSA. Show your work.

 3) You just opened a 500-mL bottle of guaifenesin and will be giving 1 tablespoon per dose. How many doses are in the bottle? In other words how many tablespoons are in 500 mL?

4) The patient's IV has Dopamine 200 mg in 250 mL D5W. The drip must run at 8 mcg/kg/min and your patient weighs 66 kg. What will the rate of mL/hr be?

 5) Five hundred mg of Keflin is within 1000 mL of 5% glucose in water.  If the IV is running at 40  gtt/min and the administration set (macrodrip tubing factor) is 15 gtt/mL, how long will it take the IV to run?
