Please help me with my nursing math homework. I can't figure out if I am doing it right. Please show work for I can understand thanksÂ
Step 1 – convert the infant’s weight to kilograms (1 kg = 2.2 lbs, 1 lb 16 oz)
Step 2 – check the literature as to the window of safety
Step 3 – calculate the safe lowest dose based on body weight (if that is what is given) and then calculate the safe highest dose based on body weight.
The safe dose is usually based on so many mg per kg body weight per day

Example: Physician orders morphine sulfate 1.8 mg IM stat.
Child weighs 79 lbs. Is the order safe?
Literature states: usual IM dose is 0.05 mg per kg

79 lb x 1 kg/2.2 lb = 35.9 kg

Safe dose should therefore be:Â (0.05 mg/kg) (35.9 kg) = 1.795 mg/dose
The ordered dose of 1.8 mg IM is safe (as for IM: round 1.795 to nearest tenth and the dose is 1.8mg).

I like to use the window of safety:Â Â [lowest safe dose <=ordered dose <=highest safe dose]

If the calculated dose is lower than the lowest safe dose, call the doctor! NOT SAFE… will not be therapeutic!

If the calculated dose is higher than the highest safe dose, call the doctor! NOT SAFE… may cause injury to the patient, perhaps death!

1) A child with severe poison ivy weighs 30 kg and Benadryl po 5 mg/kg/day is ordered q6h. Benadryl is available as a 12.5 mg/5 mL solution. What dose should be given?
2) Phenobarbital 180 mg/m2/24 hours given every eight hours is ordered for a child whose BSA (body surface area) is 0.30 m2. How much will each dose be?
3) You are to give Lidocaine 30 mcg/kg/min to a child weighing 55 lb. The piggyback contains 120 mg Lidocaine in 100 mL NS. At what rate will you set the pump?

4) The order is for amoxicillin 60 mg, po, tid for a child weighing 13 lb. The pediatric dosage range is 20-40 mg/kg/day in three equal doses. Is the dose safe?
5) Order is for Keflin 500 mg IVPB q 6 hrs for a 19 kg child. The pharmacy sends you Keflin 500 mg in 50 mL NS. What would the flow rate be if it is delivered over 30 minutes with a microdrip set? The safe child dosage is 80-160 mg/kg/da. Is the dosage you have calculated a safe dosage?
