What is a T-Account

.What is wrong with this answer to the question below??

 

What is a T-Account? On which side is the debit and on which side is the credit? Does this hold true under all circumstances?

A visual representation of the accounts in the general ledger. The account name is placed above the T, Debits will be placed on the left side just under the account name and Credits on the right side. No, it does not hold true in all circumstances that debits increase the value and credits decreases it.
