AT THE WORKPLACE

Dr. Morrison, a child psychiatrist who is in solo practice, employs one medical assistant in her office. This medical assistant is multi-skilled, like all medical assistants, and handles essentially all the administrative and clinical tasks in the office.

It is 3:00 p.m. and a parent has just arrived for a 3:30 p.m. appointment with her 10-year old daughter. The child is a new patient and was referred to Dr. Morrison by her attending physician. She has a relatively long history of combative and destructive behavior, and the referring pediatrician is seeking a psychological evaluation from Dr. Morrison. Psychotropic medication of some sort may be a viable treatment option. The medical assistant has politely asked the mother and daughter to be seated and to fill out some registration forms. The child is acting out—pulling cushions off the reception room couch, wildly ripping the pages of the magazines, whining, and kicking her mother. The behavior seems to be escalating as the mother tries to frantically control her child while, at the same time, follow the instructions of the medical assistant and fill out the registration forms.

Questions:

  • What if anything, should the medical assistant do?
  • Would it be appropriate, for example, for the medical assistant to interrupt Dr. Morrison’s current session?
  • Might this be considered a medical emergency?
