I need help on structuring this paragraph into a more professional statement and explanation. If someone can help me and also review the grammar;

 

Good morning all,

 

As of October 23^rd, 2017 we have stopped making calls for Chronic Care Assessment appointments. We are focusing on bringing in Transitional Patient and starting our new project for Gaps of Care until the end of the year. Gaps of Care will focus on the clinic to bring in patient who are missing any gaps such as; Colorectal cancer screening, Hemoglobin A1c, Urine Test, Functional Status Screening, Yearly medication Review, Pain Screening, Adult BMI screening, and breast cancer screening. Most patient will be schedule to have these gaps of care done without needing a provider in-clinic, but will need a provider to sign-off before sending it back to SCAN to input into the system. Each visit can take 5-10 minutes, while others can take 20 minutes and will need to schedule with a clinician. Once visit is complete, staff will place the routing slip for a sign off on the clinician desk. The routing slip will than return to SCAN for submission.
