This may be a simple question I don't know but this is my situation: I have a child about to be 2

Help!  This may be a simple question I don't know but this is my situation:  I have a child about to be 2.  Her dad has not had anything to do with her past 3 months old.  We were not married and we are not together.  I've taken him to court already and he now has court ordered no visitation and no contact with the child due to him on drugs.  I also have a DVO set for me.  The baby has his last name, I want it to be changed to my last name.  How hard is this to do and what are the steps to do this in the state of Kentucky?  Not sure if this is any relevance but I do not get and have never asked for any child support at all.  We are about to go back to court so I can move to different state due to better job and I may want to throw this in there with the move. Any and all info u can provide would be very helpful!
