Red was admitted to the VA hospital for sepsis. Upon discharge, Red refused to stay with his son, and insisted on returning home to the farm. Imagine you are the home health nurse making the first visit post hospitalization the next day. Assess the story about Red Yoder’s continuing journey with his chronic illness. This was just one episode in his illness journey with chronic illness. Think about the costs involved. How can you direct your efforts towards health promotion and prevention which will minimize hospital admissions and keep people healthy at home?
