Knowing what the community needs and desires are, is important to figure out what needs to change or improve on, in the local health care system. What might work for one local health care system but not work for another. I am using my own proposal of change for example. In my local community, the military does not deploy as often and when they do deploy, they see little combat since most of the missions are for transporting goods and supplies. Most of the generation is older and so are more resilient to trauma and life events. The base also is very top heavy with Generals, which effects how a serviceman wants to bring attention to him or herself. Most of the airmen on this particular base are experience and have been in the military for a while, so they are careful about exposing themselves with PTSD symptoms. I will be moving next month to a base where there is more deployment and combat operations. The base is joint Air Force and Army so the needs of the community will be different and the generation is a lot younger and income is lower so that adds more stress to the family unit which also contributes to PTSD occurrences. My mentor Doug Stiles, MSN (personal communication, April, 2017), states that “those who have been in the military longer are more resilient, know what is at stake if they seek mental health care and know how to play the system and fill out the surveys so they are not tagged with PTSD.” This verifies my proposal of change, to support and educate the spouses so that they can recognize the symptoms of PTSD and encourage the members to seek help and use the multiple resources available to them. I would love to have a few questionnaires add on the survey that each member fills out when they are at appointments so that it opens up discussion with their provider/nurse on PTSD symptoms and resources. Right now, the surveys only ask if they have been depressed lately and feel unsafe at home. I have interviewed local military PTSD clinics and the theme that resonates with them is getting help sooner than later for military that are experiencing PTSD symptoms. I have interviewed a few members who are currently seeking therapy and they agreed that spouses are pivotal to their recovery process. Healthy people 2010 website states that , "Health and quality of life rely on many community systems and factors, not simply on a well-functioning health and medical care system. Making changes within existing systems, such as improving school health programs and policies, can effectively improve the health of many in the community" ("Educational And Community-Based Programs | Healthy People 2020").
