Making a standard of care from evidence-based practice research, is not an easy task. Due to the multi-layered complex systems within healthcare, it takes planning and effort. At the local level, there is the organizational structure, leaders, teams, and the clinicians who are required to accept and adopt the EBP (Titler, 2008). Therefore, it is important to understand the facilitators and barriers within microsystems. The particular values and social environment of the healthcare unit or agency has a powerful effect on the implementation process. Moreover, there can be challenges involved with adopting the particular topic of the EBP at the local level which require strategies such as “change champions” and “multidisciplinary implementation teams” (Titler, 2008). For instance, pilot EBP practices in one unit, if successful, can expand to the rest of the organization and become the standard practice. Therefore, the local level of EBP is crucial as it takes the individual, team, and leaders of the unit or healthcare setting to support the actual change in practice.

The area of implementation of EBP can be complex and requires further research on how to best integrate research findings into practice. The local level of healthcare settings is where the EBP must be accepted and practiced. Understanding the specific settings and environments is key in the strategy used. For instance, the Dissemination and Implementation (D & I) science grant from the NIH is researching specific healthcare groups at the community level to find the best ways to teach and integrate EBP (Stevens, 2013).

According to my mentor R. Short, understanding the local healthcare opinion or views of EBP can assist in focused knowledge sharing between the interdisciplinary team in support of implementing change (R. Short, personal communication, May 08, 2017). For instance, R. Short explains that in San Diego, prior to last year, most pediatric medical care was still seen as a fee for service model and did not involve managed care. This produced little incentive for hospitals to focus on preventing ER visits and hospitalizations. Due to more research and EBP in pediatric palliative care, the focus has changed to a managed care model (R. Short, personal communication, May 08, 2017).
