After the Columbia space shuttle disaster, a commission was formed to investigate what happened. The commission reported that a piece of foam that had broken off the shuttle during liftoff caused a hole in the wing, which resulted in the disaster. The commission also noted that the culture of NASA was also at fault saying that the “organization does not provide effective checks and balances, does not have an independent safety board, and has not demonstrated the characteristics of a learning organization.” This is just one (1) example of a major failing resulting from a complex system, NASA, interacting in multiple ways. Research and examine a failure in an organization resulting from the culture of the organization. Write a three (3) page paper in which you: 1. Evaluate and discuss how the organization’s culture facilitated the failure. 2. Apply the appropriate learning disabilities and laws that describe this situation. 3. Explain the elements of the organization’s culture that will lead to similar failures in the future. 4. Recommend specific actions the organization can take to prevent future failures from occurring
