Mr. C is overweight, experiencing digestive issues and by his labs and vitals is hypertensive with diabetes and hyperlipidemia. If he continues this way of living, a domino effect of disorders will cascade into multi-symptom failures. He holds the risk of suffering a myocardial infarction, stroke, congestive heart failure, peripheral vascular disease, renal failure, developing a fatty liver, barrett’s esophagus from the peptic ulcer or even a full on gastrointestinal bleed (NIH, 2012). Nutritional deficits can also affect the skeletal and muscular system in the form of osteoarthritis.

For this case bariatric surgery would be essential in helping Mr. C to reverse his current situation and prevent further decline in his overall health. According to guidelines from the National Institute of Diabetes and Digestive and Kidney Diseases (2016), Mr. C already possesses a high BMI with serious health problems such as obesity , type 2 diabetes , heart disease, and sleep apnea making him an ideal candidate for this procedure.

Medications ordered will best fit Mr. C’s schedule and enhance compliance if given at the following times.

6am- Carafate

10:30am- Carafate, Mylanta

4:30pm- Carafate, Mylanta

9pm- Mylanta

Actual problems with Mr. C are as follows:

1. Obesity- This has been an issue since childhood and evidenced by his report of being heavy since childhood, height of 68inches and weight of 134.5kg. He works at a catalog telephone center and doesn’t mention any extracurricular activities.

2. Hypertension- A blood pressure of 172/96 can be linked to his inactivity, obesity, diet choices and hyperlipidemia.

3. Hyperlipidemia- A fasting blood glucose of 146 doesn’t only indicate that Mr. C has diabetes but also indicates a lack of healthy dietary choices. Cholesterol of 250mg/dL, triglycerides 312 mg/dL and HDL of 30 mg/dL indicates high levels of fat circulating through the vascular system which in turns creates a atherosclerosis or hardening of the vessel walls. This can create high blood pressure as it takes more force to push blood through the system and can lead to the complication of stroke or MI.

4. Sleep apnea. Most likely caused by the heavy weight creating pressure on the neck closing the airway when Mr. C tries lying down or relaxing to sleep. This can easily be reversed by weight loss.

5. Peptic Ulcer disease- A diet high in fat, cholesterol and fried foods along with the weight causes upset to the GI system which is unhealthy resulting in constant heartburn, acid reflux and general overall feeling of illness (NIH, 2012). This too can be reversed by healthy lifestyle changes including heart healthy diet and increased activity.
