Mr. C. is morbidly obese at 296.5 pounds putting him at risk for cardiac issues as well as other medical problems such as diabetes, sleep apnea and nutritional deficiencies.
The ideal cholesterol levels for a man are less than 200mg total, 40-50mg or higher for HDL and less than 100mg of LDL, Normal triglycerides are Less than 150 mg/dL.
Extra calories are turned into triglycerides and stored in fat cells for later use (American Heart Association 2016). If you eat more calories than your body needs, your triglyceride level may be high.
Is bariatric surgery an appropriate intervention? Why or why not?
Bariatric surgery would be an appropriate intervention for Mr. C. due to his health problems. Although he does not state he attempted weight loss with diet and exercise, he does state he has been overweight since childhood; leading us to believe he has had difficulty with weight and diet control all his life. Bariatric surgery does not replace healthy habits, but it might make it easier for him to consume fewer calories and become more active. He is only 32 years old and is at very high risk of mortality due to his obesity and comorbidities. According to The American Society for Bariatric Surgery, young overweight people tend to die sooner than their lean counterparts. Obesity is dangerous to health because of the associated increased prevalence of cardiovascular risk factors such as hypertension, diabetes mellitus, high triglycerides, hyperinsulinemia and low levels of high density lipoprotein (HDL) cholesterol (American Society for Bariatric Surgery). Cardiovascular risk is reduced greatly with weight loss and reduction. Bariatric surgery is not taken lightly by doctors. The patient needs to meet certain medical guidelines to qualify for weight-loss surgery. They have an extensive screening process to see if you qualify. You must also be willing to make permanent changes that lead to a healthier lifestyle and will be required to participate in long term follow up plans regarding your diet, exercise, lifestyle and health.
There are risks with any type of surgery especially with obese patients. The fact Mr. C. is obese and has sleep apnea puts him at greater risk for aspiration and respiratory complications during surgery.
Administration schedule that will be most therapeutic and acceptable to the patient.
Sucralfate/Carafate 1g or 10ml suspension (500mg/5mL)
I do not see how anyone could follow this schedule and be compliant with it. There should be another medication consideration for him that would help and be easier to follow.
Assess each of Mr. C.'s functional health patterns using the information given.
1) Pattern Nutrition and Metabolic: Altered nutrition: intake exceeds the body’s needs.
2) Pattern of Activity and Exercise: Impaired physical mobility. Has desk job. No mention of physical activity.
3) Inefficient breathing pattern related to apnea and obesity.
4) Altered gas exchange related to sleep apnea.
5) Standard of Rest and Sleep: Altered sleep patterns related to sleep apnea and obesity.
6) Self-perception and self-concept pattern: Fear, Sense of powerlessness, Situational low self-esteem, and body image disorder related to obesity.
7) Ineffective individual coping related to destructive behavior toward himself and low self-esteem, as evidence by long term obesity 100-pound weight gain in 2 years.
8) At risk for Impaired skin integrity related to obesity and poor circulation, diabetes and decreased mobility and lack of exercise.
