Spirituality has been a significant component of health and healing since ancient times in diverse cultures. Presently with science and technology having a great influence on healthcare, it is more controlled by the private sector and the government than spirituality. This has created a debate amongst healthcare practitioners and the general populace that is mentioned in the scholarly literature concerning the ethical issues of including spiritual care in nursing and medical care. This issue has affected the ideology of holistic nursing care of the individual over the health-illness continuum.

The philosophical and cultural tension between science and religion has placed nurses in a challenging relationship with patients and the interdisciplinary healthcare team members. Some patients prefer a strong spiritual component in their care when ill which can be challenging when science and technology is involved the health care system. Nurses are supposed to be informed about spirituality and implement it in their care to encourage healthy outcomes, which is different from the framework in which nurse’s practice. According to PolzerCasarez & Engebretson (2012), the culture in the United States has also added to this challenge because it endorses freedom, individualism, pluralism and tolerance and avoids passing judgment on spiritual beliefs and practices. Sometimes, it shuns any spirituality issues that are not within the secular realm of health care. Nurses are supposed to be professional in such issues and not influence judgement with their personal beliefs, morals, culture and religion.

Religion is a part of spirituality that is an established body of beliefs and practices that are part of one’s culture. With science and technology affecting the growth and development of healthcare, medicine alienated from religion addressed patient care from scientific and political perspective which is more evident in the academics. Spirituality and religion are pertinent to mental health and well-being in healthcare. Religious customs are part of cultural beliefs that entails the moral values and beliefs that influences nursing practices and conducts. “The cultural diversity of the United States and the recent communication technology and concurrent globalization has also raised many important issues regarding the contemporary position of spirituality, religion and health care. Spiritual or religious beliefs and practices are inherently relevant to health care and health behaviors’; yet the political and philosophical secularization of health care presents ethical concerns for clinicians” (PolzerCasarez & Engebretson, 2012).

Health practitioners always try to balance the relationship between healthcare and spirituality. Involving an individual’s spirituality with care addresses the whole person’s physical, emotional, social, and spiritual wellbeing. According to Carey & Cohen (2008), “Helping, fixing, and serving individuals that are ill representing three different ways of seeing life. When you help, you see life as weak. When you fix, you see life as broken. When you serve, you see life as whole. Fixing and helping may be the work of the ego, and service the work of the soul”.

Spiritual sensitivity and cultural competence is imperative to the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation that grades the nurse in decision making. Identifying a patient’s need by forming a trusting relationship, enables the nurse to identify patient’s value, morals and spirituality which in turn enables the nurse to be compassionate and incorporate spirituality in care. Spirituality in patient care leads to better health outcomes. In most individuals, their belief system and spirituality gives them a purpose and meaning to their life.
