According to an article l in The American Nurse Today a clinical nurse leader (CNL) is a master level clinician. It has been compared to the APRN because of the advance knowledge of pharmacology and physical assessments. However, the CNL is not part of a medical team and does not specialize (often referred to as generalists). CNL’s also provide education to the many professional groups that work in a facility: interdisciplinary and inter-professional. They often focus on point of care and evidence based practice, evaluate clinical interventions, work as co-coordinators of care, and work on the organizations agendas and goals (Monaghan, 2011).
The Veteran Health Administration has used CNL’s and noted a success with integrating them into the system. The need for sitters for patients with dementia has decreased, decrease in perioperative and GI patient cancellations, increase of teaching during discharge, decrease in VAPs, and increase in nursing hours. It was also noted in the article
that facilities outside of the VHA have seen a decrease in bed sores, falls, and nosocomial infections (Monaghan, 2011).
Work place advocacy promotes better patient care and empowerment to the nurses who are involved. When the voice of nurses is strengthened, and heard patient receive better care and workplace morale increases. Shared governance is an example of empowering nurses to participate in work place advocacy. Shared governance is defined as a “model of nursing practice designed to integrate core values and beliefs that professional practice embraces, as a means of achieving quality care” (Anthony, 2004). Implementing the shared governance model directly correlates with nursing retention, satisfaction, and improvement in the work environment.
