One model is the Chronic Care Model, with this they use all levels of care with those who have a number of conditions and complex chronic illnesses. The priority with this model is to manage the chronic illnesses on an outpatient basis to help minimize frequent hospitalizations and disease exacerbation. These chronic care illnesses typically need ongoing adjustments and interactions amongst their healthcare team (Improvingchroniccare.org, 2017).

An example of a complex care model that was instituted in my facility is managed by a group of advanced practice nurses who see multiple patients and frequently monitor, educate, and manage with a physician and the patient’s primary care physician. Within the past year and the group of patients that have been monitored and treated by our complex care team, they have consistently lowered all of their hospital readmissions. As they have continued to become more successful the complex care team continues to grow and make great progress.
