Root cause analysis is the belief that the best way to solve or mitigate future problems is to
address the root cause of the issue. Dealing with issues as they arise does nothing to solve the
basic flaw or problem within the project. Focusing on the root cause and directing measure
toward solving the problem is a better way to reduce the possibility of is recurrence.
A project has many phases; the final phase involves risk reduction. This provides a retrospect of
lessons learned. By openly discussing problems during a project and the actions that are taken
immediately to correct those problems, it will help start a conversation and provide ideas help to
eliminate similar occurrences in future projects.
Developing a plan for root cause analysis after a project has complications will provide a solid
foundation for mitigating risks in the future. Not all problems can be resolved during or after the
project, understanding the cause will bring to light issues that can be planned for in future
projects.

Root cause analysis was very helpful when I ran the TRICARE Operations and Patient
Administration section. The clinic was having an issue with getting outside provider's
referral results uploaded into our clinical database. This could and did delay health
treatment which could've been life threatening. Due to the severity of the issue, we had to
do a root cause analysis and we had to do it now. There were many issues but the main one
that could be changed is the processing of the referral once it was in house. In doing that,
we reduced the number of hands that had to touch the referral before it was signed,
approved and uploaded, we put a 24-hour suspense for turn-around, and created a check
and balance database to track all incoming referrals.
Through our root cause analysis, we developed and implemented plans that would continue
to help fix and monitor the referral results process. The root cause analysis was critical in
providing good health care and ultimately saved lives.
