The Therac-25 case is a classic example of ethical responsibilities going unmet. In brief, the Therac-25 was a piece of computerised medical technology released back in 1983 as a radiation therapy machine. Obviously, medical technology is supposed to aid the public good, but between 1985 and 1987 six patients died from incorrect radiation doses. The issue was a case of computer software related failure. However, was only the software to blame?
