This is a discussion question that will lead into a annotated bibliography paper. Here is the question:

Describe a Significant Nursing Clinical Issue (graded)

Reflect on your practice and identify a significant nursing clinical issue that you would like to search for evidence in online sources. This issue will be the basis for the Research Database Assignment that you will submit in Week 3. Formulate searchable, clinical questions in the PICO(T) format for your nursing clinical issue.

Remember to integrate references.

Here is the instructor's explanation of what I need:

Week 2-Thread 1-

This week I want you to identify an issue, so in the thread as the discussion question answer, you will identify the issue. This issue you will use as the issue for your annotated bibliography. In week 3, I will give you specific directions for your annotated bibliography. 

For this week let’s pick a clinical issue for this thread and explain thoroughly why this issue is significant. Include outside resources to support your issue in your discussion question answer and talk specifically about your issue, what makes it significant, what gaps in knowledge exist, how big the issue is. I am going to give you an example below so you can see one.  

I know you have your area of interest. Take the time to describe it for us. What makes it significant for you? 

Where I work there are many things I would like to change, staffing, skill mix, medication distribution, work load distribution, these are just some of the items I find as clinical issues. What are my gaps in knowledge?  

An example of a clinical issue I might use:  

1) A significant clinical issue in a perioperative setting could be: prevention of deep vein thrombosis and venous thromboembolism. Deep vein thrombosis can lead to death for hospitalized patients of all ages and remains a threat even after discharge. In fact, more than 60,000 to 100,000 Americans die from venous thromboembolism each year (Crawley, 2008). It is a preventable cause of death for patients. Let's say that I am very interested in increasing compliance with ordered mechanical devices while in the hospital in the postoperative phase of recovery.

I would like to increase care giver compliance with mechanical devices for the postoperative unit in our facility. It seems that the devices ordered are off the patients more than they are on the patients. Since the devices are a common postoperative order and not new pieces of equipment and not difficult to use, I would like to investigate as to why they are not being used as ordered and increase compliance through education on the importance of the mechanical devices in conjunction with other means. It is a significant because of the gap in knowledge in persons responsible for their application and reapplication.
