A monthly report on medication errors was presented to all units within Falls Hospital.  Although the rate has decreased, staff identified that the primary cause of medication errors continues to be interruptions while preparing and administering medications.  The nurse manager asked the staff to participate in the quality improvement process to address the problem of interruptions while RNs are preparing and admin medications.

Propose three (3) changes that can be made to prevent medication errors due to the interruptions.  Provide the rationale for each change.

Elborate on one of these changes and:

Write an outcome statement that is realistic, measureable and contains a time frame. Choose an outcome that measures the outcome of the change, not the change itself

Outline the detailed steps necessary to implement the change.

Describe how the effectiveness of change will be evaluated
