struggling with normalizing a given data table, specifically

I'm struggling with normalizing a given data table, specifically how to get from 2NF and 3NF with the data given. In all the examples I have looked up online, their 3NF looks more like my 2NF. I have attached the original given data that needs to be converted, and what I came up with for 1NF and 2NF (I haven't gotten to 3NF - this is where I'm stuck). What am I missing?
